Fall 2010/2011 Competitive Home Registration
Home Association Fee - $30.00 per player/per season (Fall/Spring)
- No fee for managers or coaches registration.
- Club/Team check or money order only-NO Cash.
- Make checks payable to CCSAI-North Dallas Chamber Soccer
- CCSAI WILL INVOICE CLUBS AS REQUESTED
- ALL OTHERS ARE REQUIRED TO PAY AT OFFICE IN PERSON

1. Players Not Returning: Delete
(do not worry about registration #)
2. New Players: Add player in GotSoccer Team Account
a. All information is required before player can be added to Official Roster
3. Coaches/Managers:
a. All coaches are required to complete an Online Background Check before being added to team

Required Forms to Complete Team Registration
1. NTSSA/USYS Competitive Membership Player Form - Click Here
(make copies,)
2. NTSSA/USYS Non-Recreation-Competitive Team Roster - Click Here
a. Signed & dated by coach
b. Complete the top portion of the form including coach and manager information
c. You do not need to fill in the roster you will have done this online in GotSoccer

Proof of Age-Birth Certificate-Passport or Drivers License (make copies)
(NO hospital/religious records will be accepted)
4. Copy of Team/Club Fact Sheet Signature page(s)
5. Risk Management/Background Check
