All required forms must be legible & submitted together.

1. Printed copy of confirmation of Club/Team Application from GotSoccer.
2. One (1) Copy of Team's Fall 2010 NTSA Official roster (signed by registrar & coach)
3. One (1) Copy of team’s Fall 2010 GotSoccer roster printed from the web page (all players must be in database in order to participate)

This application is for: U12-U19 Classic League Club Bye Teams that
have received an invitation from the CCSAI-Classic League for the
2010-2011 seasons.

Teams invoking the 75% exception rule and requesting the Bye invitation be given to team’s new club (including a 1 team club), must fill out the information below.

BYE Exception:
If (with the approval of a given team’s club) half or more of the players on a team as of April 1, 2010 freeze date, upon whose performance an invitation was based, choose instead to participate in another league, then, in order to accept the invitation, the accepting team must carry on its Fall 2010 roster a specified number of players who played for such team’s club as of the Freeze Date. Specifically, the number is equal to half or “N” plus at least one more of the leaving team’s roster as of the Freeze Date.

FAILURE TO COMPLETE PAPERWORK PROPERLY OR ON TIME MAY RESULT IN YOUR TEAM NOT BEING ABLE TO PARTICIPATE IN THE LEAGUE TRYOUTS OR LEAGUE PLAY. CONTACT THE OFFICE IF YOU HAVE ANY QUESTIONS OR PROBLEMS FILLING OUT THE FORMS, ETC.
